New Graduate Home Care Nurse (RN)

We are seeking compassionate and dedicated New Graduate Home Care Nurses (RN) to join our team. In this role, you will provide nursing care to patients in their homes, assisting them with activities of daily living and supporting their overall health and well-being.

Home Care Nurse Responsibilities:

• Provide direct nursing care to patients in their homes under the supervision of experienced nurses.
• Assist patients with personal care tasks, such as bathing, grooming, and medication management.
• Monitor patients' vital signs and assess their physical and emotional status.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Educate patients and their families about health conditions, treatment plans, and self-care practices.
• Document all patient care activities accurately and timely in electronic medical records.
• Communicate effectively with patients, families, and caregivers to provide support and guidance.
• Maintain a clean, safe, and comfortable environment for patients during visits.
• Adhere to infection control protocols, safety standards, and regulatory requirements.

Home Care Nurse Qualifications:

• Recent graduate from an accredited nursing program with an RN license in good standing.
• Strong passion for providing compassionate care to patients in their homes.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Basic computer skills and willingness to learn electronic medical records systems.
